Primary prevention with the ICD in clinical practice: not as straightforward as the guidelines suggest?
نویسندگان
چکیده
At first sight, guidelines for implantation of an implantable cardioverter defibrillator (ICD) for primary prevention of sudden cardiac death in patients with left ventricular systolic dysfunction seem unambiguous. There are clear cut-off values for ejection fraction, and functional class. However, determination of the ejection fraction itself is not unambiguous, and other risk factors for sudden death that may have a profound effect on risk are not used for decision-making. Furthermore, to obtain a clinically significant impact on survival, expected longevity is important as it can greatly compromise the benefit in elderly patients but underestimate the long-term potential of ICD therapy in younger patients. Toward more optimal use of primary prevention implantable cardioverter-defibrillators: how do we get there?
The implantable cardioverter-defibrillator (ICD) has been shown in several randomized clinical trials to improve the survival of patients with systolic heart failure (HF).1–4 As a result, practice guidelines designate ICD therapy as a class I indication in many patients with HF.5–7 Notwithstanding the evidence from randomized clinical trials and practice guidelines, several studies have demonst...

The implantable cardioverter defibrillator (ICD) represents a significant advance for the prevention of sudden cardiac death (SCD). Although clinical guidelines recommend the use of an ICD for primary prevention of SCD among specific patients with low ejection fraction, the extent of its use among those who experience SCD in the community has not been investigated. Using a prospective populatio...

متن کاملApplication of ICD guidelines and indications in a community-based academic hospital: a case series-based discussion
BACKGROUND Implantable cardioverter defibrillators (ICDs) are indeed beneficial in selected patients as evidenced by multiple large randomized controlled trials (RCTs) since 1980. A systematic method for stratification of patients and hospital-wide criteria/guidelines to ascertain appropriate device implantation became necessary. METHODS Major ICD/CRT (cardiac resynchronization therapy) clini...
